Designated Attendees:
Registration fee is waived for ONE designated component attendee.  Please mail, fax, or email the form to the national office.  Designated attendees cannot register online.  The national office will register you for PDI.  Airfare, hotel, and meals not provided by ASPAN are the responsibility of the attendee.
