With the accelerated development of more sophisticated procedures performed in outside of the traditional operating room, the demand for nurse-administered moderate sedation, as well as anesthesia services, has resulted in increased perianesthesia care of these procedural patients. This session begins with an in-depth description of the sedation continuum, from nurse-administered moderate sedation to deep and general anesthesia levels of sedation during monitored anesthesia care. Incorporating recommendations from the ASPAN Standards and the American Society of Anesthesiologists, procedural sedation across all phases of perianesthesia nursing are explored, from presedation/preanesthetic assessments, periprocedural management and monitoring recommendations to postprocedural monitoring and discharge criteria.  Analysis of adverse events and priorities of nursing care are reviewed.

This session will present the perianesthesia emergency nurse (PEN) role created and designed by an interprofessional workgroup of educators, bedside nurses, leadership, and our quality anesthesiologist. The primary function of our PEN is to respond to patients in the recovery room showing signs of clinical decompensation and assist in the evaluation, assessment, plan, interventions, and triage. It is our version of the rapid response nurse in the PACU.

No healthcare professional intends to make a medication error. Reviews of high-profile errors reveal flaws in the system that contributed to the errors. This presentation reviews progress in medication safety since the 1999 publication, ‘To Err is Human’, and describes safe medication technological strategies available today. Using a case study approach, medication errors are reviewed through the lens of how the error occurred and how use of technology and redundancy in practice mitigate risk.

#608 Intranasal Esketamine: Improving Depression and Reducing Suicide Risk

Overview

Intranasal Esketamine (IE), derived from the anesthetic ketamine, was approved for the treatment of treatment resistant depression (TRD) and major depressive disorder (MDD) with and without suicide ideation (SI) in 2019.  Based on postanesthesia expertise, perioperative care collaborated with anesthesia and behavioral health to develop a treatment service to support the treatment and monitoring needs for IE administration and increase access to care for TRD/MDD/SI patients. This intranasal esketamine service has provided a reduction in depressive symptoms and a return to functional quality of living for patients with minimal impact on the daily function of perioperative and operating room throughput.
