This lecture will explore the evidence-based journey a team of nurses took to standardize a handoff tool and process, and ultimately improve the handoff culture. After identifying large variability in reporting and an informal approach to handoff, a foundation for reforming the current handoff structure was implemented. Ultimately, the final, and most challenging step was the process we took to make handoff a cultural priority in our institution to put patient safety in the forefront of handoff in the PACU.
